1. Download the IERF Application Form (Illustration A.)

To download the file, click on the IERF icon to visit their website, click application(left side menu)-application(Scroll Down to application forms)-Physical Therapist Application and Transcript Request Form

Note: The file is in PDF Format, you must have Acrobat Reader installed in your Computer to be able to read the file. 

2. Wait for IERF Equivalency Report (Illustration B.)

To view sample, click on the IERF icon to visit their website, click application(left side menu)-sample reports-Physical Therapist report

3. Wait for the Letter of Physical Therapist Board of California (Illustration C.)
